Filling out your new patient forms before your visit is a great way to save time and lets us focus on what really matters: getting to know you and answering your questions. You can easily fill everything out online from home, at your own pace, no need to rush in a waiting room! Your info goes straight to our team, so we can check out your history and get ready for your appointment.
If you run into any issues with the forms or want to do them in person, no problem! Just bring a list of your current meds and any dental records you have, and try to arrive a few minutes early. This way, we can tackle any questions before your exam starts. Taking care of these details in advance helps keep your first appointment all about you!

Dr. Adam Kwiatkowski
Dr. Adam Kwiatkowski is a highly skilled cosmetic and implant dentist, driven by a passion for excellence. He began his journey at the University of Illinois at Chicago College of Dentistry, where he earned several accolades, including the International Congress of Oral Implantologists’ Pre-doctoral Student Achievement Award. His education continued with UIC’s Advanced Predoctoral Implant Program, and he was inducted into the prestigious Omicron Kappa Upsilon National Dental Honor Society.
Dr. Hojun Song
Dr. Hojun Song enjoys all aspects of general dentistry, with a special focus on preventive care and patient education. He believes that helping patients understand their oral health is one of the most important parts of building long-term trust and confidence. Before joining the Peakview Dentistry team, Dr. Song served as a Managing Clinical Director at Aspen Dental in Maine. He gained valuable experience in both patient care and clinical leadership.
